Accountability Forms

Please download a copy of the Accountability Form and return it to the Foundation within 60 days of receiving your grant. 

Please follow these intructions:
-Itemize all purchases made with the grant money
-Attach copies of receipts, invoices or purchase orders.
-Fill out a brief survey
-Fax or mail the form to:
